It does not form part of your contract with Southern Cross Medical Care Society. It is intended as a guide to help you understand what is covered by each module. For full policy details, including benefit terms and conditions and policy exclusions, please refer to the full **Wellbeing One and Two** policy document available on our website southerncross.co.nz/plans where you can also view the other documents that form part of the **Wellbeing One and Two** plan which may change from time to time, including the eligibility criteria, the list of unapproved healthcare services, the list of prostheses and specialised equipment, the list of Affiliated Provider-only healthcare services and the list of policy variations.
